Insurances accepted (IN-NETWORK)

All of our doctors are “In-network” providers for all the insurances that we accept. By selecting in-network providers, such as CWRC with your insurance company, you can save significantly on your out-of-pocket expenses:

  1. Negotiated fees between provider and insurance company are usually lower than those charged out-of-network. This is important to patients that have dollar limits on their infertility coverage and need to stretch those dollars over more than one treatment cycle.
  2. Patient does not incur out-of-pocket expenses for services covered except for co-pays and co-insurance.

Insurances accepted upon verification are:
Aetna/US Healthcare
GHI
POMCO
Cigna
HealthNet
UniCare
Empire   Blue Cross Blue Shield
 HIP
United Healthcare
Oxford

In addition, our affiliated anesthesiologists and urologists generally accept the same in-network benefits.  Our intention is to enable you to have a seamless experience from the point of view of maximizing your in-network health insurance coverage for your infertility treatment at CWRC.

Center of Excellence - United Resources Networks

We are pleased to announce that our Center has recently been designated as a Center of Excellence by United Resources Network.

Benefits available to members: whether or not you have coverage for fertility services under your current health insurance plan, your employer might have purchased special programs for fertility care from United Resources Network, which makes you entitled to additional benefits. Programs available through United Resources Network:

ParentSteps - a discount program for fertility services designed for members who DO NOT have infertility coverage.

Reproductive Research Services - a Centers of Excellence program for members WITH infertility coverage that provides additional educational services.

To find out if your employer has contracted any of these programs in addition to your regular health insurance, please contact your Benefits Administrator directly.

To learn more about the programs, please visit the website: https://www.urnweb.com/gateway/public/infertility/infertility.jsp

When we do not participate with your insurance (Out-of-Network)

If CWRC does not participate with your health care insurance plan you will be required to pay our fees.  Generally treatment cycle must be paid in advance.  At the time of payment we can provide you with a breakdown of the services included in each treatment cycle.  CWRC provides HCFA 1500s to patients after the services are delivered and paid in full. Please advise us if your out-of -network insurance plan has pre-authorization requirements and we will do our best to assist with providing the requested material.

CWRC is not responsible for determining pre-authorization or any other requirements for patients with out-of-network health insurance coverage.  It is the patient’s responsibility to follow up with any out-of-network insurance carrier issues.

CWRC Participates in the New York State Department of Health Infertility Demonstration Program

Patients who either do not have IVF coverage or are underinsured for IVF under their health care insurance plan may qualify for financial support through this program. Please click the link above for more details.
